Missing teeth, or edentulism, is a common problem in the United States. Edentulism reduces the patient's quality of life and makes them feel self-conscious when eating, smiling, or talking.
All-on-4 dental implants is a tooth replacement option for those who are missing most or all of their teeth. The dental team at Legacy Dental will insert four implants into the top and/or bottom jaw: two in the back and two in the front. On the day of surgery, a full arch prosthesis will be fixed to the implants. Once attached, the prosthesis can only be removed by the dentist.
A conventional dental implant is made up of 3 components:
Implant body: a titanium or zirconia screw that is implanted into the jawbone. The insert site will heal and the bone will fuse around the screw, ultimately creating a stable support for the crown. This takes 4 to 6 months
Abutment: attached to the implant body so that the dental prosthesis can be attached- may be straight or angled
Dental prosthesis: may be fixed or removable, depending on what the dentist recommends as best for your situation. With the All-on-4 implants, the prosthesis is fixed- only the dentist can remove it.

This procedure is considered an oral surgery, so you will need anesthesia. You will have a few options:
If you have high anxiety, the dentist will likely recommend conscious sedation or even general anesthesia for your treatment.
There are 2 ways that the dentist will perform the placement surgery:
Once the implants are placed, your jawbone will grow and fuse around them. This process is known as osseointegration. Depending on the quality of your bone to start with, it may take 3 to 6 months to heal completely. Typically, you will receive what is known as a provisional prosthesis the day of the surgery. Then, once healing is complete, you will be given your final prosthesis.
In some cases, the dentist will recommend delayed loading, which means you will not receive a provisional or final prosthesis until healing is complete.
As you would expect, you will likely have some discomfort following the All-on-4 dental implant surgery. Therefore, your dentist will make sure that you have painkillers to keep the pain under control. You may also have some swelling and/or bruising of your gums and face, minor bleeding/pain at surgical site, and limited ability to open your mouth.
These are common following surgery and can be controlled with your prescription painkillers and anti-inflammatory medications. However, if they get worse, you'll need to get in as soon as possible to speak with the dentist at Legacy Dental so they can address the issue.
When you visit the dental team at Legacy Dental for your evaluation to consider All-on-4 treatment, you'll want to also discuss things such as the steps for the procedure, the type of anesthesia that will be used, and any post-op instructions you need to be aware of.

Potential complications during the surgery include:
In some cases, the implant does not integrate with your jawbone, which results in implant mobility and ultimately, implant failure. This could be due to a variety of issues, including poor stability during surgery, uncontrolled diabetes, poor oral hygiene, smoking, and other factors.
Some patients develop destructive inflammation, with may affect the bone around the implants, resulting in bone loss. This is known as peri-implantitis and could be due to uncontrolled diabetes, poor oral hygiene, gum disease, bruxism, smoking, and other factors.
